Working closely with the Chief Nursing Officer, Clinical Leadership Team, and Palliative Care Medical Director, you will support the development, implementation, and ongoing advancement of palliative care services while promoting high-quality, patient- and resident-centered care. KEY RESPONSIBILITIES Palliative Care Program & Clinical Practice • Collaborate with facility leadership and staff to develop, implement, and maintain the palliative care program. • Work collaboratively with the Palliative Care Medical Director to deliver and expand palliative care services as appropriate. • Provide clinical support, including assessment, collaboration, education, and interventions, to staff caring for palliative care patients and residents. • Manage a panel of approximately 50–75 residents, determining appropriate follow-up needs and triaging new consultation requests. • Develop, review, and revise protocols, policies, procedures, order sets, and processes specific to the care needs of the targeted patient and resident population. • Collaborate with appropriate organizations and professional resources to maintain and update standards of care. • Maintain ongoing professional development and continuing education related to palliative care. • Interface with other departments to address practice issues specific to geriatric and palliative care patients and the staff who care for them. • Support compliance with applicable billing and documentation requirements in collaboration with facility leadership. Quality, Data & Program Improvement • Maintain primary responsibility for tracking palliative care consultation data. • Audit compliance with core program metrics developed to improve quality of care. • Collaborate with the Palliative Care Medical Director to routinely evaluate and report program outcomes and data. • Develop and communicate recommendations for program and quality improvements to appropriate organizational departments and leaders. • Participate in committees, project groups, and task forces as necessary to support organizational goals and objectives. Education & Collaboration • Educate physicians, staff, patients, residents, and families regarding palliative care. • Provide and promote ongoing palliative care education for physicians and staff. • Serve as a clinical resource, role model, and instructor for nursing staff and other members of the interdisciplinary care team. • Provide coaching, consultation, and education to support effective palliative care practices throughout the organization. • Promote interdisciplinary collaboration and knowledge sharing related to geriatric and palliative care. JOB DETAILS • Pay Rate: $143,000 – $170,000 annually • Hours: Full-Time • Shift Schedule: Day Shift • Days Off: Saturday & Sunday • Working Days: Monday through Friday REQUIRED QUALIFICATIONS • Current and valid California Registered Nurse (RN) license. • Bachelor of Science in Nursing (BSN) required. • Strong clinical assessment, communication, collaboration, and patient care skills. • Ability to provide clinical leadership, education, consultation, and support to interdisciplinary teams. • Ability to effectively communicate and collaborate with physicians, staff, patients, residents, families, and organizational leadership. • Ability to manage palliative care consultations, follow-up needs, and competing clinical priorities. • Ability to maintain accurate clinical, program, billing, and quality-related documentation. PREFERRED QUALIFICATIONS • Minimum of two (2) years of Registered Nurse clinical experience. • Master of Science in Nursing (MSN) with a focus in Adult-Gerontology. • Previous palliative care experience.
