Occupational Therapist - Full Time

Role Overview: Provides specialized occupational therapy services to restore and enhance patients' physical, social, and mental functions within a home health and hospice setting. Serves as a co-leader in interdisciplinary patient care teams to develop and implement treatment plans. Key Responsibilities Conducts patient evaluations to determine care needs and establishes individualized treatment and discharge plans. Administers therapy interventions using appropriate techniques and protocols. Ensures comprehensive clinical documentation within specified deadlines. Supervises assigned therapy assistants and collaborates with team members. Performs additional related duties as assigned. Qualifications & Skills Master’s degree in Occupational Therapy or Doctor of Occupational Therapy (OTD), with licensing in applicable states. Valid occupational therapy license, CPR/BLS certification, and a valid driver’s license. Strong clinical assessment, treatment planning, and documentation skills. Ability to work effectively within an interdisciplinary team.
