# Home Health Occupational Therapist - Full Time

Position Overview Seeking a full-time Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. The role involves collaborating with interdisciplinary team members, updating care plans, and ensuring optimal patient outcomes. Key Responsibilities Conduct initial and ongoing assessments of mobility, strength, and functional levels. Administer, evaluate, and interpret tests to set functional goals and measure progress. Develop and document treatment plans, progress notes, and discharge plans. Provide occupational therapy services in accordance with physician orders. Maintain accurate documentation to support billing and compliance, including OASIS assessments where applicable. Coordinate with healthcare team, patients, and caregivers, and educate them on home exercise programs. Supervise auxiliary personnel as needed and serve as a clinical resource. Qualifications Degree in Occupational Therapy from an accredited program. Current, unrestricted state license as an OT. Valid CPR certification. Preferred Skills Strong organization, communication, and attention to detail. Effective time management and relationship-building abilities. Experience in home health therapy and completion of OASIS documentation.
