Home Health Physical Therapist - Full Time

Position Overview Seeking a Full-Time Physical Therapist (PT) to provide home health care services within a designated territory. The role involves assessing patients' mobility, strength, and functional levels, and developing personalized treatment plans to facilitate recovery in their homes. Key Responsibilities Conduct initial and ongoing evaluations to determine patient needs and goals. Administer and interpret tests to evaluate progress and outcomes. Create and update written care plans, documenting patient performance and modifying treatments as necessary. Deliver physical therapy services in accordance with physician orders. Participate in discharge planning, including educating patients and caregivers on home exercises. Collaborate with healthcare team members, including physicians and nurses. Maintain thorough documentation to ensure compliance and facilitate seamless care. Supervise other clinical personnel as applicable. Complete assessments such as OASIS where permitted by regulation. Qualifications Degree in Physical Therapy from an accredited program. Current, unrestricted state license as a Physical Therapist. Valid CPR certification. Preferred Skills Strong organization, communication, and time management skills. Attention to detail and relationship-building abilities. Experience in home health therapy and OASIS documentation.
