Home Health Physical Therapist - Full Time

Position Overview Seeking a Physical Therapist (PT) to assess, plan, and deliver therapy interventions aligned with physician orders, focusing on patient mobility, strength, and functional goals. You will perform assessments, interpret tests, develop treatment plans, and document progress to ensure optimal patient outcomes. Responsibilities include providing therapy services, modifying care plans as needed, and collaborating with healthcare teams, patients, and caregivers to ensure effective discharge planning and education. Supervisory duties may include overseeing PTA, HHA, or OTA personnel, and completing OASIS assessments where permitted. You will maintain compliance with policies, standards, and regulations, ensuring effective communication within the team.
