Transition Care Navigator

Responsibilities: Identifies patients that meet criteria for the Readmission Reduction Program and Bundled Program for Care Improvements program Introduce the programs to the patients and/or families. Assigns patients to the Transition Care Managers and Coordinators. Coordination of patient telehealth use, if applicable. Registers patients for home visits. Conducts a minimum of 40 follow-up telephone calls to the patients Documents the assessments for every phone call Report to the assigned TCC or TCM for any issues. EDUCATION: High school diploma and/or GED EXPERIENCE: Medical Assistant with at least one year of experience in a doctor’s office and/or acute-care facility. LICENSURE / QUALIFICATIONS: Certification as a Medical Assistant preferred. Spanish-speaking preferred. SHIFT: Monday through Friday 8am to 4:30pm. May require weekend shifts and on-call.
