Nurse (Clinical/Case Management)

About the Position: This position is located at Naval Hospital Jacksonville. This is a Direct Hire Solicitation. Salary negotiation may be available for those candidates who are new to Federal service. Incentives may be available for highly qualified applicants. Coordinates services needed by patients with a variety of complexities and provides intervention when the patient deviates from the anticipated path of progression. Conducts a comprehensive assessment of beneficiary’s health needs to develop a plan of care. Serves as liaison between the patient and the health care professionals. Conduct assessment and identify needs to formulate care plans for patients with a variety of complexhealth concerns, which includes researching, analyzing, testing, and evaluating information, uncommon situations, or unpredictable conditions. Coordinates services, referrals, and appointments for each patient under case management services. Completes follow up assessments in the electronic record with updated information of the patient's current physical and emotional condition. Attends daily multi-disciplinary team meetings to address current issues, and improve the overall process, and identify potential case management patients review of the multiple databases and points of contacts available for the specified population. Basic Requirement for Nurse (Clinical/Case Management): Degree: A graduate or higher-level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. (Note: You must attach a copy of your transcripts.) Licensure: A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination. Foreign Degree: Graduates of foreign nursing schools must have completed the Commission on Graduates of Foreign Nursing Schools Certification Program. The certification program consists of a credentials evaluation, the CGFNS Qualifying Exam, and evidence of passing one of the approved English Language Proficiency examinations. A copy of the CGFNS Certificate is required. Exemption: You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017, or before. In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below: Education/Experience: A diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program and possession of at least one full year of professional nursing experience (equivalent to the GS-9 grade level or above) which includes: coordinating patient care, and educating patients or staff, and assisting with developing patient care plans. OR Education: Successful completion of two and a half years of progressively higher-level graduate education leading towards a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field. EXEMPTION: You are exempt from the qualification requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before.
