James A. Haley Veterans' Hospital is looking for a Social Worker Program Coordinator who will be assigned to the Community Residential Care (CRC) Program. The CRC serves as program coordinator and is administratively responsible for the CRC program. The CRC Coordinator manages the daily operation of the program and provides direct patient care services in the program area to a small caseload.
The CRC Coordinator manages and controls the use of data related to these programs regarding day-to-day functioning and program evaluation. The CRC Coordinator, plans, organizes, and directs administrative, operational, patient and personnel activities for the service conjunction with the Geriatrics and Extended Care Section Chief of Social Work Service. The Social Worker will adhere to the National Association of Social Work Code of Ethics, all local social work policies and/or SOPs and maintain an unrestricted, unsuspended license to practice social work. Additional duties include, but are not limited to; Responsibilities include the daily operation of the program, developing policies and procedures for program operation and preparing reports and statistics for facility, VISN, and national use. The CRC Coordinator is capable of practicing at the independent level and has specialized knowledge of practice typically related to a particular diagnosis or population. CRC Coordinator should have at least 2 years in an area of specialized social work practice and licensed or hold certification in a state at the independent practice level. A qualified supervisor or equivalent in the state of licensure, that can teach, coach, train, and mentor others in the profession of social work. Provides administrative oversight of program and administers information and analytical systems to evaluate and enhance the quality of service provided to patients. Recruits and screens potential CRC facilities, develops the application for the CRC sponsors, coordinates and retains record of VA inspections of CRC facilities and directs CRC staff duties. The CRC Coordinator also develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction. Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. The CRC Coordinator provides assessment, treatment, and planning for a complex population of patients which reflect the age-specific, psychosocial, and functional needs of the patient (e.g., geriatric, spinal cord, geriatric and mental health population). The CRC Coordinator visits each CRC facility monthly for oversight and compliance with standards. Demonstrates knowledge of VA and community resources to meet the complex psychosocial needs of this unique population of patients and to ensure they are referred to the least restrictive environment that meets the level of care needed. The CRC Coordinator has knowledge of Federal and State laws and regulations that pertain to the care of vulnerable adults in community residential homes. The CRC Coordinator has responsibility for coordinating initial fire and safety inspections of these homes to ensure the safety of the veteran population served. Thereafter, the CRC Coordinator coordinates the inspection of the homes on an annual basis. Performs work related to the review, monitoring, analysis, planning and assessment of the Community Residential Care Program. Responsible for key project activities related to the planning, development, and implementation of the project component. A substantial portion of the workload directed by this position is carried out at one or more locations that are physically removed from the main unit under conditions that make day-to-day coordination complex to administer. Performs a wide range of services and provides technical assistance to state, local, private agencies, and universities. Technical assistance provided is based on monitoring activities and program reviews, and the development and analysis of information. In addition, the CRC Coordinator uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. The CRC Coordinator performs program analysis, development, review, research, design, and contact work as possible with various agencies (public, DOD, State, etc.). Independently deals with crises and effectively uses their problem-solving skills to assist the veteran, family and staff. Monitors homes with unannounced visits at least once per month. In addition, CRC Coordinator communicates with multidisciplinary teams, which may include home care teams (Home Based Primary Care, Spinal Cord Injury-HG and Mental Health Intensive Case Management) regarding veterans' contentment with home and care received in the home. Addresses any circumstance that may arise during weekly team meeting or sooner depending on issue. • CPTSPR Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. Experience: The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment and are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure: Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Assignments: For all assignments above the full performance level, the higher- level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
