Travel Physical Therapist - Home Health

Role: The Physical Therapist (PT) is responsible for assessing, planning, and providing rehabilitation services to patients, establishing individualized treatment programs, and documenting patient progress. Responsibilities: Develops care plans, delivers skilled interventions according to physician orders, evaluates patient needs, collaborates with multidisciplinary teams, and educates patients and families on therapy methods. Qualifications & Skills: Must hold a Doctorate or Bachelor's degree in Physical Therapy, pass the NPTE, have a minimum of one year's experience, hold or be in the process of obtaining state licensure, and possess current CPR certification. Key Duties: Conduct patient assessments, administer treatments, modify care plans as needed, document patient conditions, and collaborate with healthcare team members to achieve therapy goals. Additional Requirements: Graduation from an accredited school and compliance with facility-specific requirements.
