Physical Therapist (PT)

We are looking for a Physical Therapist to join our home health team in a full-time capacity. We are looking to build a team of committed team players who want to practice at a high level. We embrace a small company culture with the resources of a larger company encompassing many aspects of care including Hospice, Home Health, primary care, assisted living. We empower clinicians to be true case managers we do not dictate treatment timelines and support them creating unique Plans of care with their patients. We are innovating across all domains; clinical practice, marketing, EMR, administration and this a unique opportunity to be a part of a growing team and have a part in creating a practice an elevated practice in Home Health.
