MDS Coordinator (Registered Nurse/RN)

Role Overview: The RN MDS Coordinator is responsible for coordinating, completing, and submitting accurate interdisciplinary MDS Assessments, CAAs, and Care Plans in compliance with CMS RAI Manual Regulations and applicable laws. Qualifications: Requires an associate’s or bachelor’s degree in nursing, an active RN license, and at least two years of nursing experience, preferably in geriatrics. CPR certification is also required. Key Responsibilities Coordinate and assist with timely completion of MDS Assessments, CCAs, and Care Plans Report patient condition changes identified through assessments Educate staff on CMS RAI Manual updates and documentation requirements Review and validate assessments and ensure proper system submissions before billing Monitor CMS Reports for assessment timeliness and implement process improvements Participate in interdisciplinary meetings and perform staff nurse functions as needed Maintain confidentiality, adhere to company policies, and uphold professionalism and integrity Demonstrate excellent customer service, teamwork, and communication skills Assist with patient evacuation and ensure safety protocols are followed Skills and Attributes: Advanced practice knowledge, independent decision-making, strong organizational skills, and the ability to multitask in a fast-paced environment.
