Rehabilitation Registered Nurse

Registered Nurse (Rehab Nurse) provides compassionate, faith-based skilled rehab nursing care for seniors. Key responsibilities include performing nursing assessments, developing and updating individualized care and rehab plans, administering medications and treatments, and coordinating with therapy and interdisciplinary teams to promote safe recovery and best outcomes. Additional Duties Include Educating residents and families on care needs and rehab goals Documenting assessments, interventions, and outcomes accurately in the EMR Supporting CNAs and other staff with clinical guidance Upholding a mission of dignity, respect, and person-centered care Maintaining collaborative, values-driven standards of practice Qualifications/Skills: Must be a licensed Registered Nurse with strong clinical judgment, assessment and care-planning skills, medication/treatment administration competence, clear communication, accurate documentation in an EMR, and an empathetic approach to resident and family education.
