Help us save lives and make a difference.&nbsp; Earn More with Shift Differentials Maximize your earnings with additional hourly pay: +$2/hr -&gt; Monday-Thursday Nights +$3/hr -&gt; Friday-Sunday Nights +$2/hr -&gt; Saturday-Sunday Days What You'll Do: &nbsp; The&nbsp;Case Manager&nbsp;I&nbsp;facilitates&nbsp;admission intakes, treatment planning,&nbsp;discharge planning, obtaining collateral and&nbsp;assisting&nbsp;in the legal process&nbsp;and&nbsp;initiating&nbsp;personalized care for individuals who are in crisis and stabilizing from crisis.&nbsp;Works collaboratively&nbsp;with&nbsp;the&nbsp;interdisciplinary&nbsp;treatment&nbsp;team to&nbsp;coordinate&nbsp;care&nbsp;and&nbsp;discharge&nbsp;planning&nbsp;for&nbsp;Individuals, their family members, clinics, local providers, and/or other referral sources.&nbsp;This position will carry a caseload and act as&nbsp;ambassador&nbsp;to community partners.&nbsp; Educates and informs individuals of&nbsp;consents&nbsp;for treatment and authorizations for use and disclosures.&nbsp; Participates&nbsp;regularly&nbsp;in Interdisciplinary Treatment Team meetings.&nbsp; Collaborates with the Treatment Team to develop and implement an individualized person-centered Treatment&nbsp;Plan&nbsp;that&nbsp;is&nbsp;appropriate&nbsp;for&nbsp;brief&nbsp;intervention.&nbsp;Complete&nbsp;the&nbsp;signed&nbsp;Treatment&nbsp;Plan&nbsp;and&nbsp;review with each&nbsp;individual and&nbsp;any other involved parties.&nbsp;&nbsp; Gather collateral from the&nbsp;individual&nbsp;and involved parties&nbsp;to provide a thorough assessment of what an&nbsp;individual’s’&nbsp;needs are for treatment and discharge.&nbsp; Actively&nbsp;coordinate&nbsp;care&nbsp;and provide referrals&nbsp;for&nbsp;individuals&nbsp;to&nbsp;ensure&nbsp;services&nbsp;are&nbsp;coordinated&nbsp;with&nbsp;both&nbsp;facility&nbsp;staff&nbsp;and external providers.&nbsp; Identify&nbsp;and address gaps in service needs for participating parties in service delivery to the&nbsp;individual and&nbsp;make&nbsp;appropriate recommendations&nbsp;to&nbsp;meet Individuals’&nbsp;needs upon discharge.&nbsp; Collaborate&nbsp;with&nbsp;Interdisciplinary&nbsp;Treatment&nbsp;Team&nbsp;to&nbsp;engage,&nbsp;monitor,&nbsp;and&nbsp;communicate&nbsp;with&nbsp;the&nbsp;individual,&nbsp;ensuring that safety is&nbsp;maintained&nbsp;while meeting the individual’s needs.&nbsp; Maintain&nbsp;the&nbsp;clinic&nbsp;record,&nbsp;including&nbsp;documentation&nbsp;of&nbsp;activities&nbsp;performed&nbsp;as&nbsp;part&nbsp;of&nbsp;the&nbsp;service&nbsp;delivery process.&nbsp;Obtain signatures from&nbsp;individuals&nbsp;regarding&nbsp;treatment, as necessary. Document all services and Individuals’&nbsp;activities in the Electronic Health Record (EHR)&nbsp;regarding&nbsp;brief interventions, coordination of care, discharge planning, treatment planning,&nbsp;and any other pertinent&nbsp;information.&nbsp; Maintain a working knowledge of the&nbsp;Virginia&nbsp;Involuntary Commitment process.&nbsp;&nbsp; Maintain&nbsp;a&nbsp;working&nbsp;knowledge&nbsp;of&nbsp;the&nbsp;DBHDS and DMAS behavioral&nbsp;health system. Participate&nbsp;in&nbsp;individual&nbsp;and&nbsp;group&nbsp;supervision&nbsp;as&nbsp;required.&nbsp;Participate&nbsp;in&nbsp;ongoing&nbsp;education,&nbsp;including&nbsp;in-services, training, and other activities to&nbsp;maintain&nbsp;and improve competency.&nbsp;Demonstrate competency through post-testing, skill observation, and performance as assessed by direct supervisor.&nbsp; Complete all required training, attend monthly meetings, and&nbsp;maintain&nbsp;required competencies.&nbsp; Coordinates civil commitment process with all necessary stakeholders.&nbsp; Provide daily programming/groups.&nbsp; Performs&nbsp;all&nbsp;other&nbsp;duties&nbsp;as&nbsp;assigned. What You'll Bring: A Bachelor’s degree in behavioral health or related field&nbsp; Knowledge of the Commonwealth of Virginia behavioral health system of care and resources within it Qualified Mental Health Professional (QMHP) The Company has a mandatory vaccination policy.
