Providers (Primary Care, Subspecialty): $2,500 Care Coordinators (Brooklyn & Florida Hubs): $1,000 Medical Assistants and Techs: $1,000 Office Staff: $500 How to Refer: We know you've got a lot on your plate, so we've made this as low-lift as possible. Enter Your Information: We just need your name, email, ant your preference for receiving updates. Submit: Hit "Submit application" and you're done! That's it!
