Role Overview As a Community Health Worker, you¿ll serve as a trusted liaison between healthcare providers, social services, and the community. Your role is critical in promoting access to healthcare, improving health equity, and connecting individuals to essential services. You¿ll advocate for patients, provide culturally appropriate health education, and support patient-centered care, making a direct impact on the well-being of underserved populations. What You Will Do Promote access to healthcare services and improve health equity outcomes. Connect individuals to social services and primary care resources. Provide culturally appropriate health education and support to patients. Address social determinants of health to ensure patient-centered care. Collaborate with clinical teams and community partners to achieve cost-savings and quality objectives. What You Will Need Required Qualifications High school diploma or G.E.D. equivalent. Certification as a Community Health Worker or willingness to become certified within six months of hire. Preferred Qualifications Previous experience as a Community Health Worker. Experience conducting health education or working with underserved populations.
