Home Health Registered Nurse - Full Time

Join the Home Health team as an Admissions Registered Nurse to provide personalized, skilled nursing care that promotes patient independence and positive outcomes. Responsibilities Deliver and document care based on individualized plans and physician orders. Perform patient assessments and develop tailored care strategies. Educate patients and families on medications, symptom management, and lifestyle changes. Train and support caregivers for safe, effective home care. Provide hands-on care such as wound management, infusions, catheter care, pain control, and post-operative support. Collaborate with physicians and care teams to ensure coordinated, high-quality outcomes. Qualifications And Skills Active RN license in the state of residence. Current CPR certification. Valid driver’s license, reliable transportation, and current car insurance. Preferred: at least 1 year of home health, med-surge, ER, or hospice nursing experience.
