This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Independently implement treatment modalities, provide counseling and/or supportive groups for Veterans and families. Provide consultation to other treatment team and staff members regarding psychosocial needs of Veterans and/or their families and the impact of the identified psychosocial problems on the Veteran's health care planning, compliance with treatment, and discharge planning. Serve on committees, work groups, and task forces at the facility as deemed appropriate by the supervisor and Chief of SWS. Responsible for furthering one's own professional growth through continuing education, for ensuring continuing education appropriate to the area of assignment Provides assistance with the orientation and learning of less experienced Social Workers. Provides clinical supervision to social worker's seeking independent license and/or accept asocial work trainee. Demonstrates knowledge and skill in the use of software applications for drafting documents and data management as well as other computer systems in use by the VHA. Maintains a level of productivity and quality consistent with the complexity of the assignment and consistent with Social Work standards, Long Term Care Institute standards, and facility standards. Consults with their supervisor as needed and with colleagues to develop knowledge and apply agency policies and practices. Documents social work interventions and activities in the Veteran's clinical record utilizing CPRS, in accordance with facility policy, and ensure appropriate hand off when transferring of patient care. Provides care coordination for Veteran's discharged from the geropsychiatric unit at 1 week and 1-month post discharge to help with adjustment to a new setting and answer questions/provide advice about potential or real behavioral concerns and help prevent the need for readmission. Contacts social services at institutional settings (i.e. ACHs) for Veteran's discharged from the geropsychiatric unit at 1 week to help with adjustment to a new setting and answerquestions/provide advice about potential or real behavioral concerns and help prevent the need for readmission. Prepares Veterans and their families for dealing with major neurocognitive disorders by providing training or consult services to SAVAHCS providers on the importance of and need for, advanced care planning for Veterans with, or at risk for, major neurocognitive disorders. Education: Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for master's degree in social work. Licensure: Candidate must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).Grade Determinations: In addition to the basic requirements, applicants must have one year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Formal training in dementia care best practices (e.g., caregiver interventions, behavior management), with continuing education in geriatrics, neurocognitive disorders, or caregiver support. Experience delivering staff training (nurses, social workers, providers) on dementia communication techniques, safety planning, resources, and caregiver support; comfortable presenting at huddles, in services, and safety forums.
