Nurse (Case Manager)

Provides case management services to individuals and communities, ensuring coordinated care, compliance with public health and clinical guidelines, evaluation of outcomes. Develops care plans and serves as liaison to patients, providers, care teams. This position reports to the Director of Communicable Disease Prevention and Control at the Chinle Service Unit, Division of Nursing Services. Leads and coordinates contact investigations and outbreak responses, conducting field visits to assess, screen, treat, and manage individuals, families, and communities. Plans, implements, and evaluates communicable disease programs; coLlaborates with Tribal, local, state, and federal partners; and coordinates outreach clinics and prevention activities. Maintains disease registries, analyzes trends, tracks testing and treatment outcomes, and prepares required surveillance reports to support program effectiveness and public health action. Provides education and training to staff, patients, and community partners; offers clinical guidance; leads care coordination efforts; and supports quality improvement and evidence-based practices. BASIC REQUIREMENT(S): Education A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education(external link), Council on Accreditation of Nurse Anesthesia Educational Programs(external link), Accreditation Commission for Midwifery Education(external link), or an accrediting body recognized by the U.S. Department of Education(external link) at the time the degree was obtained. Degree from Foreign Nursing School:Official certification from the Commission on Graduates of Foreign Nursing Schools (external link) is required for individuals who graduated from foreign nursing schools. Licensure For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Experience The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above grade level, many positions may require experience in a specialty area of nursing. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below- MINIMUM QUALIFICATIONS: In addition to the above requirements, applicants must meet one of the following requirements: Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or At least one full year of professional nursing experience (equivalent to the GS-11 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level GS-11 in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: One year of specialized experience equivalent to at least the GS-11 level in the Federal service that includes independently managing and coordinating communicable disease case management programs; leading complex disease investigations and outbreak response activities; interpreting and applying laboratory and epidemiological data to guide clinical and public health decisions; ensuring timely treatment, partner services, and follow-up in accordance with established guidelines (e.g., CDC protocols); and collaborating with Tribal, local, State, and Federal partners to implement prevention and control strategies.This experience also includes analyzing surveillance data and trends to inform program actions, developing and evaluating public health initiatives, providing technical guidance and training to clinical and public health staff, and coordinating multidisciplinary care and services across clinical and community settings.
