# Lead Medical Instrument Technician (EKG)

The duties for this position include coordinating, supervising, as well as operating and monitoring diagnostic instruments and equipment associated with cardiac testing. He / She provides care based on the physical, psychosocial, educational, safety and related criteria appropriate to the age (adult and geriatric populations) and medical status of the client. This entails knowledge of human growth and development, ability to assess and interpret age response to treatment modalities.
The incumbent performs the following cardiac testing procedures: Measurement and analysis of cardiac electrocardiogram waveforms. Identification and analysis of electrocardiogram intervals, segments, and complexes. Application of oxygen therapy and analysis of pulse oximetry measurement. Performance and analysis of standard 12 lead electrocardiogram. Downloading of Holter testing recording and interpretation. Data input and editing into the Muse System. Data input and troubleshooting of the GE Aquision Module (Holter monitoring, Event monitoring, Stress Testing, ECG testing). Performance of non-invasive blood pressure measurements with the identification of normal/abnormal values and initiation of the treatment plan. Performance and analysis of vital signs. Perform the duties of the Facility ECG Administrator as defined Coordination with Biomedical Engineering on the creation and maintenance of an ECG cart location table with mutually agreed-upon naming conventions. If any ECG on the ECG Management System does not have a location, then report the issue to Biomedical Engineering immediately for resolution. Correction of patient demographic errors on ECG studies sent to the ECG Management System. Ensuring accurate routing of ECG studies to Cardiologist or other appropriate LIP over- reader in-baskets for interpretation. Inputting new Cardiologist physicians, or other appropriate LIP over-reader users, into the ECG Management System and inactivate old users as they enter/leave the department. DO NOT DELETE old users. Initial setting up of Cardiologist or other appropriate LIP over-reader user profiles in the ECG Management System. On a monthly basis, verifying the workflow configuration on the ECG Management System including management of location ranges for user profiles, configuration of physician in-baskets, and oversight of filters applied to the system. Documentation of verification is required. On a weekly basis, running database reports on the ECG Management System to query for unread/unconfirmed ECG studies, mismatched ECG studies, or other failures to properly route ECG studies for physician or other appropriate LIP over-read. On a weekly basis, communicating with the nursing staff in off-site clinical locations with ECG carts, to verify that the carts are functioning appropriately and that ECG studies are being appropriately routed to the ECG Management System. Documentation of verification is required. In coordination with biomedical engineering, verifying weekly that ECG studies are not being automatically confirmed in the ECG Management System without an official over-read by a Cardiologist or other LIP who is credentialed and privileged to interpret ECGs Perform other duties as assigned. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Medical Instrument Technicians must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Licensure or Certification. Licensure or Certification is not required for this occupation; however, it is strongly desirable at GS-6 or above as evidence of possession of the essential knowledge, skills, and abilities. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-8 Experience: At least 1 year of experience comparable to the next lower grade level, which is directly related to the duties of the position to be filled, and must fully meet the KSAs at that level. This would be experience which provided knowledge of advanced specialized procedures, the pharmacology related to this occupation, and knowledge of related acute disorders and diseases and their effects on organs and methods of treatment. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs designated by an asterisk ( ): Demonstrated Knowledge, Skills, and Abilities. KSAs 1, 2, 3, and 4 are appropriate for all positions at this level. KSAs 5 and 6 are to be used in accordance with their appropriateness for the specific assignment. 1. Knowledge of pharmacology related to specialized procedures. 2. Ability to provide briefings and orientations to hospital staff including physicians. 3. Knowledge of a variety of related acute disorders and diseases, their effects on organs, and methods of treatment. 4. Ability to perform more complex procedures such as SAECG (Signal Averaged ECG), Tilt table monitoring, electrophysiology monitoring, and event monitoring. 5. Ability to provide, or provide for, staff development and training. 6. Ability to manage and supervise employees. Certification. No certification is required; however, it is desirable that employees at this level have BCLS (Basic Cardiac Life Support) and ACLS (Advanced Cardiac Life Support) certifications. Assignments. Medical instrument technicians (EKG) at this level perform complicated examinations or treatments for which there are no standard instructions or procedures. They utilize knowledge of anatomy and physiology including an in depth understanding of the functioning of the major systems and internal organs to interpret requests and to recognize the need for additional tests or a different position. Knowledge of pharmacology including the classification and administration of drugs, patient responses, and common dosages as related to this specialty is required. The technician has the ability to perform Signal Averaged ECG, Tilt table monitoring, electrophysiology monitoring, and event monitoring. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and range of variety, and be performed by the incumbent at least 25% of the time. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, lifting more than 10 pounds, and pushing. Transferring patients and objects may be required. The MIT may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The MIT may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The MIT must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
