# Rehab Liaison- Full Time (RN or LSW)

The Rehab Liaison is responsible for marketing and initial assessment/screening of potential patients that require physical, occupational, and/or speech therapy in acute inpatient rehab at internal/external facilities. The Rehab Liaison will establish relationships with referral sources including case management and physicians on a regular basis. He/She will participate in interdisciplinary care planning, coordination of service delivery, utilization review, and discharge planning. Coordinates with the rehab staff in maintaining CMS compliance. Actively participates in monthly Leadership meetings. Requirements: RN/ NJ License or LSW (Licensed Social Worker) Master's degree in Social Work or BSN degree With 1-2 years related experience, hospital setting preferred Shift: 8:30am- 5:00pm We are an Equal Opportunity Employer and a member of the Employer Partnership of the Armed Forces.
